General

A yellowish tinge of the skin or sclera may indicate accumulation due to impaired renal excretion and the need to discontinue therapy. The decline in renal function associated with advanced age should be kept in mind.

NOTE: Patients should be informed that Phenazopyridine (Carmurit T) HCl produces a reddish-orange discoloration of the urine and may stain fabric. Staining of contact lenses has been reported.

Laboratory Test Interaction

Due to its properties as an azo dye, Phenazopyridine (Carmurit T) HCl may interfere with urinalysis based on spectrometry or color reactions.

Carcinogenesis, Mutagenesis, Impairment Of Fertility

Long-term administration of Phenazopyridine (Carmurit T) HCl has induced neoplasia in rats (large intestine) and mice (liver). Although no association between Phenazopyridine (Carmurit T) HCl and human neoplasia has been reported, adequate epidemiological studies along these lines have not been conducted.

Pregnancy Category B

Reproduction studies have been performed in rats at doses up to 50 mg/kg/day and have revealed no evidence of impaired fertility or harm to the fetus due to Phenazopyridine (Carmurit T) HCl. There are, however, no adequate and well controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, this drug should be used during pregnancy only if clearly needed.

Nursing Mothers

No information is available on the appearance of Phenazopyridine (Carmurit T) HCl, or its metabolites in human milk.

What happens if I miss a dose of Phenazopyridine (Carmurit T)?

Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at your next regularly scheduled time. Do not take extra medicine to make up the missed dose.

Acute

Signs of acute overdosage with Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) may appear following ingestion of 1 gram or more of the drug and include nausea, vomiting, dizziness, headaches, mental depression, confusion and bone marrow depression.

Treatment consists of gastric lavage and general supportive measures. Acidification of the urine will increase renal elimination of Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet). Peritoneal dialysis is not effective and hemodialysis is only moderately effective in eliminating the drug.

Chronic

Use of Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) at high doses and/or for extended periods of time may cause bone marrow depression manifested as thrombocytopenia, leukopenia and/or megaloblastic anemia. If signs of bone marrow depression occur, Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) should be discontinued and the patient should be given leucovorin; 5 to 15 mg leucovorin daily has been recommended by some investigators.

What should I avoid while taking Trimethoprim (Carmurit T)?

Avoid exposure to sunlight or tanning beds. Trimethoprim (Carmurit T) can make you sunburn more easily. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors.

Trimethoprim (Carmurit T) warnings

Serious hypersensitivity reactions have been reported rarely in patients on Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) therapy. Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) has been reported rarely to interfere with hematopoiesis, especially when administered in large doses and/or for prolonged periods.

The presence of clinical signs such as sore throat, fever, pallor or purpura may be early indications of serious blood disorders. Complete blood counts should be obtained if any of these signs are noted in a patient receiving Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) and the drug discontinued if a significant reduction in the count of any formed blood element is found.

What should I discuss with my healthcare provider before taking Trimethoprim (Carmurit T)?

Some medical conditions may interact with Trimethoprim (Carmurit T). Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:

Some MEDICINES MAY INTERACT with Trimethoprim (Carmurit T). Tell your health care provider if you are taking any other medicines, especially any of the following:

This may not be a complete list of all interactions that may occur. Ask your health care provider if Trimethoprim (Carmurit T) may interact with other medicines that you take. Check with your health care provider before you start, stop, or change the dose of any medicine.

Trimethoprim (Carmurit T) precautions

General

Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) should be given with caution to patients with possible folate deficiency. Folates may be administered concomitantly without interfering with the antibacterial action of Trimethoprim (Carmurit T).

Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) should also be given with caution to patients with impaired renal or hepatic function.

Carcinogenesis, Mutagenesis, Impairment of Fertility

Carcinogenesis

Long-term studies in animals to evaluate carcinogenic potential have not been conducted with Trimethoprim (Carmurit T).

Mutagenesis

Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) was demonstrated to be nonmutagenic in the Ames assay. In studies at two laboratories, no chromosomal damage was detected in cultured Chinese hamster ovary cells at concentrations approximately 500 times human plasma levels; at concentrations approximately 1000 times human plasma levels in these same cells, a low level of chromosomal damage was induced at one of the laboratories. No chromosomal abnormalities were observed in cultured human leukocytes at concentrations of Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) up to 20 times human steady-state plasma levels. No chromosomal effects were detected in peripheral lymphocytes of human subjects receiving 320 mg of Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) in combination with up to 1600 mg of sulfamethoxazole per day for as long as 112 weeks.

Impairment of Fertility

No adverse effects on fertility or general reproductive performance were observed in rats given Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) in oral dosages as high as 70 mg/kg/day for males and 14 mg/kg/day for females.

Pregnancy

Teratogenic Effects

Pregnancy Category C. Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) has been shown to be teratogenic in the rat when given in doses 40 times the human dose. In some rabbit studies, the overall increase in fetal loss (dead and resorbed and malformed conceptuses) was associated with doses six times the human therapeutic dose.

While there are no large, well-controlled studies on the use of Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) in pregnant women, Brumfitt and Pursell, Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) (Trimethoprim (Carmurit T) tablet) tablet) is known to be substantially excreted by the kidney, and the risk of toxic reactions to this drug may be greater in patients with impaired renal function. Because elderly patients are more likely to have decreased renal function, care should be taken in dose selection, and it may be useful to monitor potassium concentrations and to monitor renal function by calculating creatinine clearance.
