Inhaler: Provides short-acting (4-6 hrs) bronchodilation with fast onset (within 5 min) in reversible airways obstruction.

It is particularly suitable for the relief and prevention of asthma symptoms. It should be used to relieve symptoms when they occur, and to prevent them in those circumstances recognised by the patient to precipitate an asthma attack (eg, before exercise or unavoidable allergen exposure).

Accuhaler: Management of asthma, bronchospasm and/or reversible airways obstruction.

Inhaler/Accuhaler: Relief and prevention of asthma symptoms. It should be used to relieve symptoms when they occur and to prevent them in those circumstances recognised by the patient to precipitate an asthma attack (eg, before exercise or unavoidable allergen exposure).

It is particularly valuable as a relief medication in mild, moderate or severe asthma, provided that reliance on it does not delay the introduction and use of regular inhaled corticosteroid therapy.

Nebule: Routine management of chronic bronchospasm unresponsive to conventional therapy and in the treatment of acute severe asthma (eg, status asthmaticus).

Injection/IV Infusion: Relief of severe bronchospasm associated with asthma or bronchitis and for the treatment of status asthmaticus. Short-term management of uncomplicated premature labour; to arrest labour between 22 and 37 weeks of gestation in patients with no medical or obstetric contraindication to tocolytic therapy.

MDI: Each actuation delivers Derihaler 100 100 mcg. It is a suspension aerosol for oral inhalation supplied in a pressurized container.

Derihaler 100 dosage

Derihaler 100 for oral use as a means of extending the broncho adults and children over 12 years - 2.4 mg 3-4 times / day; if necessary, the dose may be increased to 8 mg 4 times / day. Children aged 6-12 years - 2 mg 3-4 times / day; children 2-6 years - 1-2 mg 3 times / day.

Derihaler 100 for inhalation use dose depends on the applied dosage form, frequency of use depends on the testimony and the clinical situation.

As a tocolytic agent used as IV infusion in dose of 1-2 mg.

Derihaler 100 interactions

Ventolin and nonselective β-blocking drugs eg, propranolol, should not usually be prescribed together. Ventolin is not contraindicated in patients under treatment with monoamine oxidase inhibitors (MAOIs).

IV Infusion: Halogenated Anaesthetics: Owing to the additional antihypertensive effect, there is increased uterine inertia with risk of haemorrhage; in addition, serious ventricular rhythm disorders due to increased cardiac reactivity, have been reported on interaction with halogenated anaesthetics. Treatment should be discontinued, whenever possible, at least 6 hrs before any scheduled anaesthesia with halogenated anaesthetics.

Corticosteroids: Systemic corticosteroids are frequently given during premature labour to enhance foetal lung development. There have been reports of pulmonary oedema in women concomitantly administered with β-agonists and corticosteroids.

Corticosteroids are known to increase blood glucose and can deplete serum potassium, therefore concomitant administration should be undertaken with caution with continuous patient monitoring owing to the increased risk of hyperglycaemia and hypokalaemia.

Antidiabetics: The administration of β-agonists is associated with a rise of blood glucose, which can be interpreted as an attenuation of anti-diabetic therapy; therefore individual antidiabetic therapy may need to be adjusted.

Potassium Depleting Agents: Owing to the hypokalaemic effect of β-agonists, concurrent administration of serum potassium depleting agents known to exacerbate the risk of hypokalaemia eg, diuretics, digoxin, methyl xanthines and corticosteroids, should be administered cautiously after careful evaluation of the benefits and risks with special regard to the increased risk of cardiac arrhythmias arising as a result of hypokalaemia.

Tremors (particularly the hands), palpitations, tachycardia, hypertension, headache, nervousness, peripheral vasodilation, and rarely muscle cramps have been observed after administration of salbutamol aerosol inhalation. Potentially serious hypokalemia has also been reported.

Cases of urticaria, angioedema, rash, anaphylaxis, paradoxical bronchospasm, epistaxis, cough, hoarseness, oropharyngeal edema, heartburn, hypotension, arrhythmias (including atrial fibrillation, supraventricular tachycardia and extrasystoles), and collapse have also been noted after the use of inhaled salbutamol.

Salbutamol can also cause angina, vomiting, vertigo, CNS stimulation, hyperactivity (in children), unusual taste, and dryness or irritation of the oropharynx.

Derihaler 100 contraindications

Patients with a history of hypersensitivity to Derihaler 100 or to any of the components of Ventolin.

Accuhaler: Although IV salbutamol and occasional salbutamol tablets are used in the management of premature labour uncomplicated by conditions eg, placenta praevia, ante-partum haemorrhage or toxaemia of pregnancy, inhaled salbutamol preparations are not appropriate for managing premature labour. Salbutamol presentations should not be used for threatened abortion.

Ventolin accuhaler are contraindicated in patients with severe milk-protein allergy.

IV Infusion: Ventolin solution for IV infusion is contraindicated in the following conditions: Any condition at a gestational age <22 weeks; as a tocolytic agent in patients with preexisting ischaemic heart disease or those patients with significant risk factors for ischaemic heart disease; threatened abortion during the 1st and 2nd trimester; any condition of the mother or foetus in which prolongation of the pregnancy is hazardous eg, severe toxaemia, intrauterine infection, vaginal bleeding resulting from placenta praevia, eclampsia or severe preeclampsia, placental abruption or cord compression; intrauterine foetal death, known lethal congenital or lethal chromosomal malformation.

Ventolin solution for IV infusion is also contraindicated in any preexisting medical conditions with which a betamimetic would have an untoward effect eg, pulmonary hypertension and cardiac disorders eg, hypertrophic obstructive cardiomyopathy or any type of obstruction of the left ventricular outflow tract eg, aortic stenosis.
