What is Dynacin capsules?

Dynacin capsules is used to treat pimples and red bumps (non-nodular inflammatory lesions) that occur with moderate to severe acne vulgaris in patients 12 years of age and older.

Dynacin capsules capsules are used to treat bacterial infections in many different parts of the body. It is also used to treat anthrax infection and other infections in patients who cannot receive penicillins.

Dynacin capsules belongs to the class of medicines known as tetracycline antibiotics. It works by killing bacteria or preventing their growth. However, Dynacin capsules will not work for colds, flu, or other virus infections.

Dynacin capsules is available only with your doctor's prescription.

Dynacin capsules® Pellet-Filled Capsules are indicated in the treatment of the following infections due to susceptible strains of the designated microorganisms:

Rocky Mountain spotted fever, typhus fever and the typhus group, Q fever, rickettsialpox and tick fevers caused by rickettsiae.

Respiratory tract infections caused by Mycoplasma pneumoniae.

Lymphogranuloma venereum caused by Chlamydia trachomatis.

Psittacosis (Ornithosis) due to Chlamydia psittaci.

Trachoma caused by Chlamydia trachomatis, although the infectious agent is not always eliminated, as judged by immunofluorescence.

Inclusion conjunctivitis caused by Chlamydia trachomatis.

Nongonococcal urethritis, endocervical, or rectal infections in adults caused by Ureaplasma urealyticum or Chlamydia trachomatis.

Relapsing fever due to Borrelia recurrentis.

Chancroid caused by Haemophilus ducreyi.

Plague due to Yersinia pestis.

Tularemia due to Francisella tularensis.

Cholera caused by Vibrio cholerae.

Campylobacter fetus infections caused by Campylobacter fetus.

Brucellosis due to Brucella species (in conjunction with streptomycin).

Bartonellosis due to Bartonella bacilliformis.

Granuloma inguinale caused by Calymmatobacterium granulomatis.

Dynacin capsules is indicated for the treatment of infections caused by the following gram-negative microorganisms when bacteriologic testing indicates appropriate susceptibility to the drug:

Escherichia coli.

Enterobacter aerogenes.

Shigella species.

Acinetobacter species.

Respiratory tract infections caused by Haemophilus influenzae.

Respiratory tract and urinary tract infections caused by Klebsiella species.

Dynacin capsules® Pellet-Filled Capsules are indicated for the treatment of infections caused by the following gram-positive microorganisms when bacteriologic testing indicates appropriate susceptibility to the drug:

Upper respiratory tract infections caused by Streptococcus pneumoniae.

Skin and skin structure infections caused by Staphylococcus aureus. (Note: Dynacin capsules is not the drug of choice in the treatment of any type of staphylococcal infection.)

When penicillin is contraindicated, Dynacin capsules is an alternative drug in the treatment of the following infections:

Uncomplicated urethritis in men due to Neisseria gonorrhoeae and for the treatment of other gonococcal infections.

Infections in women caused by Neisseria gonorrhoeae.

Syphilis caused by Treponema pallidum subspecies pallidum.

Yaws caused by Treponema pallidum subspecies pertenue.

Listeriosis due to Listeria monocytogenes.

Anthrax due to Bacillus anthracis.

Vincent’s infection caused by Fusobacterium fusiforme.

Actinomycosis caused by Actinomyces israelii.

Infections caused by Clostridium species.

In acute intestinal amebiasis, Dynacin capsules may be a useful adjunct to amebicides.

In severe acne, Dynacin capsules may be useful adjunctive therapy.

Oral Dynacin capsules is indicated in the treatment of asymptomatic carriers of Neisseria meningitidis to eliminate meningococci from the nasopharynx. In order to preserve the usefulness of Dynacin capsules in the treatment of asymptomatic meningococcal carriers, diagnostic laboratory procedures, including serotyping and susceptibility testing, should be performed to establish the carrier state and the correct treatment. It is recommended that the prophylactic use of Dynacin capsules be reserved for situations in which the risk of meningococcal meningitis is high.

Oral Dynacin capsules is not indicated for the treatment of meningococcal infection.

Although no controlled clinical efficacy studies have been conducted, limited clinical data show that oral Dynacin capsules hydrochloride has been used successfully in the treatment of infections caused by Mycobacterium marinum.

To reduce the development of drug-resistant bacteria and maintain the effectiveness of Dynacin capsules® (Dynacin capsules hydrochloride) Pellet-Filled Capsules and other antibacterial drugs, Dynacin capsules® (Dynacin capsules hydrochloride) Pellet-Filled Capsules should be used only to treat or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. When culture and susceptibility information are available, they should be considered in selecting or modifying antibacterial therapy. In the absence of such data, local epidemiology and susceptibility patterns may contribute to the empiric selection of therapy.

Acute intestinal amebiasis: Adjunctive therapy to amebicides in the treatment of acute intestinal amebiasis

Acne:

Oral (immediate release) and IV:

Adjunctive therapy for the treatment of severe acne

Oral (extended-release):

Treatment of only inflammatory lesions of non-nodular moderate to severe acne vulgaris in patients 12 years and older

Off Label Uses

Cellulitis (purulent) due to community-acquired MRSA

Based on the Infectious Diseases Society of America (IDSA) Clinical Practice Guidelines for the Treatment of Methicillin-Resistant Staphylococcus Aureus Infections in Adults and Children, Dynacin capsules is effective and recommended in the outpatient management of purulent cellulitis due to community-acquired methicillin-resistant S. aureus (MRSA).

Leprosy

Based on the National Hansen's Disease Program (NHDP) treatment recommendations, Dynacin capsules, in combination with other agents, is an effective and recommended alternative agent for treatment of lepromatous (multibacillary) and tuberculoid (paucibacillary) leprosy.

Nocardiosis

Data from a limited number of patients studied suggest that Dynacin capsules, with or without other concomitant antimicrobials, may be beneficial for the treatment of nocardiosis

Prosthetic Joint Infection

Based on the Infectious Diseases Society of America (IDSA) Diagnosis and Management of Prosthetic Joint Infection: Clinical Practice Guideline, Dynacin capsules (based on in vitro susceptibility) given for indefinite chronic oral antimicrobial suppression of prosthetic joint infections is an effective and recommended treatment option. Dynacin capsules is one of the preferred agents for Staphylococci (oxacillin-resistant) infections and is an alternative treatment for infections caused by Cutibacterium spp.

Rheumatoid arthritis

Data from older controlled studies and a meta-analysis have demonstrated a beneficial effect on laboratory parameters and modest clinical benefit in patients with rheumatoid arthritis (RA). Some data suggest that Dynacin capsules may be effective in patients with recent-onset disease to reduce the total steroid dose needed.

The American College of Rheumatology (ACR) updated guideline for the treatment of RA did not include Dynacin capsules due to its infrequent use in RA and lack of new data since prior publication.

Dynacin capsules hydrochloride is a yellow, crystalline powder; sparingly soluble or soluble in water, slightly soluble in alcohol, practically insoluble in chloroform and in ether, dissolves in solutions of alkali hydroxides and carbonates.

Dynacin capsules hydrochloride is 4-7, bis (dimethylamino)-1, 4, 4a, 5, 5a, 6, 11, 12a-octahydro-3,10,12, 12a-tetrahydroxy- 1,11-dioxo-2-naphthacenecarboxamide monohydrochloride.

It has a molecular formula of C23H27N3O7 HCl. Molecular weight is 493.94.

The recommended dosage of Dynacin capsules® is approximately 1 mg/kg daily for 12 weeks. Higher doses have not shown to be of additional benefit in the treatment of inflammatory lesions of acne, and may be associated with more acute vestibular side effects.

Dynacin capsules® Tablets may be taken with or without food. Ingestion of food along with Dynacin capsules® may help reduce the risk of esophageal irritation and ulceration.

In patients with renal impairment, the total dosage should be decreased by either reducing the recommended individual doses and/or by extending the time intervals between doses.

Anticoagulants

Because tetracyclines have been shown to depress plasma prothrombin activity, patients who are on anticoagulant therapy may require downward adjustment of their anticoagulant dosage.

Penicillin

Since bacteriostatic drugs may interfere with the bactericidal action of penicillin, it is advisable to avoid giving tetracycline-class drugs in conjunction with penicillin.

Methoxyflurane

The concurrent use of tetracycline and methoxyflurane has been reported to result in fatal renal toxicity.

Antacids and Iron Preparations

Absorption of tetracyclines is impaired by antacids containing aluminum, calcium or magnesium and iron-containing preparations.

Low Dose

Oral Contraceptives

In a multi-center study to evaluate the effect of Dynacin capsules on low dose oral contraceptives, hormone levels over one menstrual cycle with and without Dynacin capsules 1 mg/kg once-daily were measured. Based on the results of this trial, Dynacin capsules-related changes in estradiol, progestinic hormone, FSH and LH plasma levels, of breakthrough bleeding, or of contraceptive failure, cannot be ruled out. To avoid contraceptive failure, female patients are advised to use a second form of contraceptive during treatment with Dynacin capsules.

Drug/Laboratory Test Interactions

False elevations of urinary catecholamine levels may occur due to interference with the fluorescence test.

Do not use this medicine if you are pregnant. It could harm the unborn baby or cause permanent tooth discoloration later in life.

Dynacin capsules can make birth control pills less effective. Ask your doctor about using a non hormone method of birth control (such as a condom, diaphragm, spermicide) to prevent pregnancy while using Dynacin capsules.

Dynacin capsules passes into breast milk and may affect bone and tooth development in a nursing baby. Do not take this medication without telling your doctor if you are breast-feeding a baby.

Children should not take Dynacin capsules. Dynacin capsules can cause permanent yellowing or graying of the teeth in children younger than 8 years old.

Avoid exposure to sunlight or tanning beds. Dynacin capsules can make you sunburn more easily. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors.

Do not take iron supplements, multivitamins, calcium supplements, antacids, or laxatives within 2 hours before or after taking Dynacin capsules. These products can make Dynacin capsules less effective.

Take this medication for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antibiotics.

Throw away any unused medicine after the expiration date on the label has passed. Using expired Dynacin capsules can cause damage to your kidneys.
