What is Insulin Purified Regular Pork?
Insulin Purified Regular Pork (insulin inhalation) is a rapid-acting form of human insulin that is inhaled through the mouth. Insulin is a hormone that works by lowering levels of glucose (sugar) in the blood. Insulin Purified Regular Pork inhalation is a fast-acting insulin that starts to work about 15 minutes after inhalation, peaks in about 1 hour, and keeps working for 2 to 4 hours.
Insulin Purified Regular Pork inhalation is used to improve blood sugar control in adults with diabetes mellitus.
If you have type 1 diabetes, you will also need to use a long-acting injectable insulin.
If you have type 2 diabetes, Insulin Purified Regular Pork may be the only medicine you need to control your blood sugar. However, your doctor may prescribe a long-acting injectable insulin or a diabetes medicine you take by mouth.

Insulin Purified Regular Pork® is a rapid acting inhaled insulin indicated to improve glycemic control in adult patients with diabetes mellitus.
Limitations of Use:
- Insulin Purified Regular Pork is not a substitute for long-acting insulin. Insulin Purified Regular Pork must be used in combination with long-acting insulin in patients with type 1 diabetes mellitus.
- Insulin Purified Regular Pork is not recommended for the treatment of diabetic ketoacidosis.
- The safety and efficacy of Insulin Purified Regular Pork in patients who smoke has not been established. The use of Insulin Purified Regular Pork is not recommended in patients who smoke or who have recently stopped smoking.
How should I use Insulin Purified Regular Pork?
Use Insulin Purified Regular Pork innolets as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- An extra patient leaflet is available with Insulin Purified Regular Pork innolets. Talk to your pharmacist if you have questions about this information.
- Use Insulin Purified Regular Pork innolets within 30 to 60 minutes before a meal, as directed by your doctor.
- If you will be using Insulin Purified Regular Pork innolets at home, a health care provider will teach you how to use it. Be sure you understand how to use Insulin Purified Regular Pork innolets. Follow the procedures you are taught when you use a dose. Contact your health care provider if you have any questions.
- You may use Insulin Purified Regular Pork innolets in an insulin pump if you are directed to do so by your doctor. IF you are using an insulin pump do NOT dilute Insulin Purified Regular Pork innolets or mix it together with any other type of insulin.
- Insulin Purified Regular Pork innolets should be clear and colorless. Do not use Insulin Purified Regular Pork innolets if it contains particles, is cloudy or discolored, or if the vial is cracked or damaged.
- If you are mixing Insulin Purified Regular Pork innolets with another insulin, draw Insulin Purified Regular Pork innolets into the syringe first. Inject the dose immediately after mixing, as directed by your doctor.
- Use the proper technique taught to you by your doctor. Inject deep under the skin, NOT into a vein or muscle.
- Injection sites within an injection area (abdomen, thigh, upper arm) must be rotated from one injection to the next.
- Be sure you have purchased the correct insulin. Insulin comes in a variety of containers, including vials, cartridges, and pens. Make sure that you understand how to properly measure and prepare your dose. If you have any questions about measuring and preparing your dose, contact your doctor or pharmacist for information.
- Insulin Purified Regular Pork innolets begins lowering blood sugar within 30 to 60 minutes after an injection. The effects last up to 4 to 12 hours.
- Keep this product, as well as syringes and needles, out of the reach of children and pets. Do not reuse needles, syringes, or other materials. Ask your health care provider how to dispose of these materials after use. Follow all local rules for disposal.
- It is very important to follow your insulin regimen exactly. Do NOT miss any doses. Ask your doctor for specific instructions to follow in case you ever miss a dose of insulin.

Note: Concentrated U-500 regular insulin is indicated only in patients requiring more than 200 units of insulin per day.
Off Label Uses
Cadaveric organ recovery (hormonal resuscitation)
Data from three retrospective cohort studies of brain-dead donors who successfully donated organs suggests that the use of intravenous regular insulin (and dextrose) given concomitantly with levothyroxine or liothyronine, methylprednisolone, and vasopressin may be beneficial for hemodynamically unstable brain-dead donors to increase the quantity and quality of organs available for transplantation.

Important Administration Instructions
- Prescribe Insulin Purified Regular Pork ONLY to patients who require more than 200 units of insulin per day.
- Train patients on proper use and injection technique before initiating Insulin Purified Regular Pork. Training reduces the risk of administration errors such as needle sticks and dosing errors.
- To avoid serious dosing errors for patients using the vial presentation, determine the type of syringe that the patient will use to deliver the prescribed dose of Insulin Purified Regular Pork, instruct the patient on how to correctly draw the prescribed dose of Insulin Purified Regular Pork into the particular syringe, and confirm that the patient has understood these instructions and can correctly draw the prescribed dose of Insulin Purified Regular Pork with their syringe.
- Instruct patients to always check the insulin label before administration to confirm the correct insulin product is being used.
- Inspect Insulin Purified Regular Pork visually for particulate matter and discoloration. Only use Insulin Purified Regular Pork if the solution appears clear and colorless.
- Instruct patients to inject Insulin Purified Regular Pork subcutaneously into the thigh, upper arm, abdomen, or buttocks.
- Rotate injection sites within the same region from one injection to the next to reduce the risk of lipodystrophy.
- DO NOT administer Insulin Purified Regular Pork intravenously or intramuscularly.
- DO NOT dilute or mix Insulin Purified Regular Pork with any other insulin products or solutions.
Dosing Instructions
- Instruct patients to inject Insulin Purified Regular Pork subcutaneously usually two or three times daily approximately 30 minutes before meals.
- Individualize and titrate the dosage of Insulin Purified Regular Pork based on the patient's metabolic needs, blood glucose monitoring results, and glycemic control goal.
- Dosage adjustments may be needed with changes in physical activity, changes in meal patterns (i.e., macronutrient content or timing of food intake), changes in renal or hepatic function, changes in medications or during acute illness to minimize the risk of hypoglycemia or hyperglycemia.
Delivery of Insulin Purified Regular Pork using the Insulin Purified Regular Pork Disposable Prefilled KwikPen Device
- DO NOT perform dose conversion when using the Insulin Purified Regular Pork KwikPen. The dose window of the Insulin Purified Regular Pork KwikPen shows the number of units of Insulin Purified Regular Pork to be injected and NO dose conversion is required.
- DO NOT transfer Insulin Purified Regular Pork from the Insulin Purified Regular Pork KwikPen into a syringe for administration as overdose and severe hypoglycemia can occur.
- The Insulin Purified Regular Pork KwikPen is for single patient use only.
Delivery of Insulin Purified Regular Pork using a U-100 Insulin Syringe or a Tuberculin Syringe
- U-100 insulin and tuberculin syringes are not scaled to measure Insulin Purified Regular Pork insulin. When using a U-100 insulin syringe or a tuberculin syringe to deliver HUMULIN U-500, a conversion step is REQUIRED to ensure the correct amount of Insulin Purified Regular Pork is drawn up in the syringe. Failure to follow this conversion step will result in failure to draw up the correct amount of Insulin Purified Regular Pork in the syringe and will result in serious dosing errors.
- Markings on the U-100 syringe refer to units of less concentrated insulin products (i.e., U-100 insulin products) and NOT units of Insulin Purified Regular Pork. If U-100 insulin syringes are used, prescribers or healthcare workers MUST CONVERT the prescribed dose of Insulin Purified Regular Pork into a U-100 insulin syringe “unit marking” to determine the correct amount of Insulin Purified Regular Pork to draw up in the syringe. Prescribers or healthcare workers MUST DETERMINE that patients can draw up the correct amount of Insulin Purified Regular Pork in the syringe before prescribing and dispensing.
- To convert from the Insulin Purified Regular Pork prescribed dose to a U-100 insulin syringe “unit marking”, divide the prescribed dose of Insulin Purified Regular Pork by 5. The resulting number is the “unit marking” on a U-100 insulin syringe to which the patient should draw up Insulin Purified Regular Pork. For example, if a patient is prescribed 100 units of Insulin Purified Regular Pork, then the patient should draw up Insulin Purified Regular Pork to the 20 “unit mark” on a U-100 insulin syringe (i.e., 100 divided by 5).
- Markings on the tuberculin syringe refer to volume (mL) and NOT units of Insulin Purified Regular Pork. If tuberculin syringes are used, prescribers or healthcare workers MUST CONVERT the prescribed dose of Insulin Purified Regular Pork into a tuberculin syringe “volume marking” in mL to determine the correct amount of Insulin Purified Regular Pork to draw up in the syringe. Prescribers or healthcare workers MUST DETERMINE that patients can draw up the correct amount of Insulin Purified Regular Pork in the syringe before prescribing and dispensing.
- To convert from the Insulin Purified Regular Pork prescribed dose to a tuberculin syringe “volume marking” in mL, divide the prescribed dose of Insulin Purified Regular Pork by 500. The resulting number is the “volume (mL) marking” on a tuberculin syringe to which the patient should draw up Insulin Purified Regular Pork. For example, if a patient is prescribed 100 units of Insulin Purified Regular Pork, then the patient should draw up Insulin Purified Regular Pork to the 0.2 mL “volume mark” on a tuberculin syringe (i.e., 100 divided by 500).

Drugs That May Increase The Risk Of Hypoglycemia
The risk of hypoglycemia associated with Insulin Purified Regular Pork use may be increased with antidiabetic agents, ACE inhibitors, angiotensin II receptor blocking agents, disopyramide, fibrates, fluoxetine, monoamine oxidase inhibitors, pentoxifylline, pramlintide, propoxyphene, salicylates, somatostatin analogs (e.g., octreotide), and sulfonamide antibiotics. Dose adjustment and increased frequency of glucose monitoring may be required when Insulin Purified Regular Pork is co-administered with these drugs.
Drugs That May Decrease The Blood Glucose Lowering Effect Of Insulin Purified Regular Pork
The glucose lowering effect of Insulin Purified Regular Pork may be decreased when co-administered with atypical antipsychotics (e.g., olanzapine and clozapine), corticosteroids, danazol, diuretics, estrogens, glucagon, isoniazid, niacin, oral contraceptives, phenothiazines, progestogens (e.g., in oral contraceptives), protease inhibitors, somatropin, sympathomimetic agents (e.g., albuterol, epinephrine, terbutaline) and thyroid hormones. Dose adjustment and increased frequency of glucose monitoring may be required when Insulin Purified Regular Pork is co-administered with these drugs.
Drugs That May Increase Or Decrease The Blood Glucose Lowering Effect Of Insulin Purified Regular Pork
The glucose lowering effect of Insulin Purified Regular Pork may be increased or decreased when co-administered with alcohol, beta-blockers, clonidine, and lithium salts. Pentamidine may cause hypoglycemia, which may sometimes be followed by hyperglycemia. Dose adjustment and increased frequency of glucose monitoring may be required when Insulin Purified Regular Pork is co-administered with these drugs.
Drugs That May Affect Hypoglycemia Signs And Symptoms
The signs and symptoms of hypoglycemia may be blunted when beta-blockers, clonidine, guanethidine, and reserpine are co-administered with Insulin Purified Regular Pork.

Clinical Trials Experience
Because clinical trials are conducted under widely varying designs, the incidence of adverse reactions reported in one clinical trial may not be easily compared to the incidence reported in another clinical trial, and may not reflect what is observed in clinical practice.
The data described below reflect exposure of 3017 patients to Insulin Purified Regular Pork and include 1026 patients with type 1 diabetes and 1991 patients with type 2 diabetes. The mean exposure duration was 8.17 months for the overall population and 8.16 months and 8.18 months for type 1 and 2 diabetes patients, respectively. In the overall population, 1874 were exposed to Insulin Purified Regular Pork for 6 months and 724 for greater than one year. 620 and 1254 patients with type 1 and type 2 diabetes, respectively, were exposed to Insulin Purified Regular Pork for up to 6 months. 238 and 486 patients with type 1 and type 2 diabetes, respectively, were exposed to Insulin Purified Regular Pork for greater than one year (median exposure = 1.8 years). Insulin Purified Regular Pork was studied in placebo and active-controlled trials (n = 3 and n = 10, respectively).
The mean age of the population was 50.2 years and 20 patients were older than 75 years of age. 50.8% of the population were men; 82.6% were White, 1.8% were Asian, and 4.9% were Black or African American. 9.7% were Hispanic. At baseline, the type 1 diabetes population had diabetes for an average of 16.6 years and had a mean HbA1c of 8.3%, and the type 2 diabetes population had diabetes for an average of 10.7 years and had a mean HbA1c of 8.8%. At baseline, 33.4% of the population reported peripheral neuropathy, 32.0% reported retinopathy and 19.6% had a history of cardiovascular disease. Table 1 shows common adverse reactions, excluding hypoglycemia, associated with the use of Insulin Purified Regular Pork in the pool of controlled trials in type 2 diabetes patients. These adverse reactions were not present at baseline, occurred more commonly on Insulin Purified Regular Pork than on placebo and/or comparator and occurred in at least 2% of patients treated with Insulin Purified Regular Pork.

Weight Gain
Weight gain may occur with some insulin therapies, including Insulin Purified Regular Pork. Weight gain has been attributed to the anabolic effects of insulin and the decrease in glycosuria. In a clinical trial of patients with type 2 diabetes, there was a mean 0.49 kg weight gain among Insulin Purified Regular Pork-treated patients compared with a mean 1.13 kg weight loss among placebo-treated patients.
Antibody Production
Increases in anti-insulin antibody concentrations have been observed in patients treated with Insulin Purified Regular Pork. Increases in anti-insulin antibodies are observed more frequently with Insulin Purified Regular Pork than with subcutaneously injected mealtime insulins. Presence of antibody did not correlate with reduced efficacy, as measured by HbA1c and fasting plasma glucose, or specific adverse reactions.

Take care not to let your blood sugar get too low. Low blood sugar (hypoglycemia) can occur if you skip a meal, exercise too long, drink alcohol, or are under stress. Symptoms include headache, hunger, weakness, sweating, tremors, irritability, or trouble concentrating. Carry hard candy or glucose tablets with you in case you have low blood sugar. Other sugar sources include orange juice and milk. Be sure your family and close friends know how to help you in an emergency.
Also watch for signs of blood sugar that is too high (hyperglycemia). These symptoms include increased thirst, increased urination, hunger, dry mouth, fruity breath odor, drowsiness, dry skin, blurred vision, and weight loss. Your blood sugar will need to be checked often, and you may need to adjust your insulin dose.
Never share an injection pen or cartridge with another person. Sharing injection pens or cartridges can allow disease such as hepatitis or HIV to pass from one person to another.
Insulin is only part of a complete program of treatment that may also include diet, exercise, weight control, foot care, eye care, dental care, and testing your blood sugar. Follow your diet, medication, and exercise routines very closely. Changing any of these factors can affect your blood sugar levels.
Do not change the brand of insulin or syringe you are using without first talking to your doctor or pharmacist. Some brands of Insulin Purified Regular Pork and syringes are interchangeable, while others are not. Your doctor and/or pharmacist know which brands can be substituted for one another.
