Levodopa (Madopar 250mg) has not been formally assigned to a pregnancy category. However, some of the commercially available formulations of Levodopa (Madopar 250mg)-carbidopa have been assigned to pregnancy category C by the FDA. Animal studies have revealed evidence of visceral and skeletal malformations in rabbits at doses 5 to 20 times the maximum recommended human dose of Levodopa (Madopar 250mg)-carbidopa. There are no controlled data in human pregnancy. Two reports have described three patients who received Levodopa (Madopar 250mg) or Levodopa (Madopar 250mg)-carbidopa throughout pregnancy without evidence of fetal harm. Levodopa (Madopar 250mg) should only be given in human pregnancy when benefit outweighs risk.

One study has reported that single oral doses of Levodopa (Madopar 250mg) administered to pregnant women result in decreased plasma renin activity, decreased plasma aldosterone and decreased plasma prolactin.

There are no data on the excretion of Levodopa (Madopar 250mg) into human milk. Because of the potential for serious adverse reactions in nursing infants, a decision should be made to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
