Levodopa (Madopar Dual Release) has not been formally assigned to a pregnancy category. However, some of the commercially available formulations of Levodopa (Madopar Dual Release)-carbidopa have been assigned to pregnancy category C by the FDA. Animal studies have revealed evidence of visceral and skeletal malformations in rabbits at doses 5 to 20 times the maximum recommended human dose of Levodopa (Madopar Dual Release)-carbidopa. There are no controlled data in human pregnancy. Two reports have described three patients who received Levodopa (Madopar Dual Release) or Levodopa (Madopar Dual Release)-carbidopa throughout pregnancy without evidence of fetal harm. Levodopa (Madopar Dual Release) should only be given in human pregnancy when benefit outweighs risk.

One study has reported that single oral doses of Levodopa (Madopar Dual Release) administered to pregnant women result in decreased plasma renin activity, decreased plasma aldosterone and decreased plasma prolactin.

There are no data on the excretion of Levodopa (Madopar Dual Release) into human milk. Because of the potential for serious adverse reactions in nursing infants, a decision should be made to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
