What is Mino Riemser?
Mino Riemser is used to treat pimples and red bumps (non-nodular inflammatory lesions) that occur with moderate to severe acne vulgaris in patients 12 years of age and older.
Mino Riemser capsules are used to treat bacterial infections in many different parts of the body. It is also used to treat anthrax infection and other infections in patients who cannot receive penicillins.
Mino Riemser belongs to the class of medicines known as tetracycline antibiotics. It works by killing bacteria or preventing their growth. However, Mino Riemser will not work for colds, flu, or other virus infections.
Mino Riemser is available only with your doctor's prescription.
Mino Riemser indications
Mino Riemser® Pellet-Filled Capsules are indicated in the treatment of the following infections due to susceptible strains of the designated microorganisms:
Rocky Mountain spotted fever, typhus fever and the typhus group, Q fever, rickettsialpox and tick fevers caused by rickettsiae.
Respiratory tract infections caused by Mycoplasma pneumoniae.
Lymphogranuloma venereum caused by Chlamydia trachomatis.
Psittacosis (Ornithosis) due to Chlamydia psittaci.
Trachoma caused by Chlamydia trachomatis, although the infectious agent is not always eliminated, as judged by immunofluorescence.
Inclusion conjunctivitis caused by Chlamydia trachomatis.
Nongonococcal urethritis, endocervical, or rectal infections in adults caused by Ureaplasma urealyticum or Chlamydia trachomatis.
Relapsing fever due to Borrelia recurrentis.
Chancroid caused by Haemophilus ducreyi.
Plague due to Yersinia pestis.
Tularemia due to Francisella tularensis.
Cholera caused by Vibrio cholerae.
Campylobacter fetus infections caused by Campylobacter fetus.
Brucellosis due to Brucella species (in conjunction with streptomycin).
Bartonellosis due to Bartonella bacilliformis.
Granuloma inguinale caused by Calymmatobacterium granulomatis.
Mino Riemser is indicated for the treatment of infections caused by the following gram-negative microorganisms when bacteriologic testing indicates appropriate susceptibility to the drug:
Escherichia coli.
Enterobacter aerogenes.
Shigella species.
Acinetobacter species.
Respiratory tract infections caused by Haemophilus influenzae.
Respiratory tract and urinary tract infections caused by Klebsiella species.
Mino Riemser® Pellet-Filled Capsules are indicated for the treatment of infections caused by the following gram-positive microorganisms when bacteriologic testing indicates appropriate susceptibility to the drug:
Upper respiratory tract infections caused by Streptococcus pneumoniae.
Skin and skin structure infections caused by Staphylococcus aureus. (Note: Mino Riemser is not the drug of choice in the treatment of any type of staphylococcal infection.)
When penicillin is contraindicated, Mino Riemser is an alternative drug in the treatment of the following infections:
Uncomplicated urethritis in men due to Neisseria gonorrhoeae and for the treatment of other gonococcal infections.
Infections in women caused by Neisseria gonorrhoeae.
Syphilis caused by Treponema pallidum subspecies pallidum.
Yaws caused by Treponema pallidum subspecies pertenue.
Listeriosis due to Listeria monocytogenes.
Anthrax due to Bacillus anthracis.
Vincent’s infection caused by Fusobacterium fusiforme.
Actinomycosis caused by Actinomyces israelii.
Infections caused by Clostridium species.
In acute intestinal amebiasis, Mino Riemser may be a useful adjunct to amebicides.
In severe acne, Mino Riemser may be useful adjunctive therapy.
Oral Mino Riemser is indicated in the treatment of asymptomatic carriers of Neisseria meningitidis to eliminate meningococci from the nasopharynx. In order to preserve the usefulness of Mino Riemser in the treatment of asymptomatic meningococcal carriers, diagnostic laboratory procedures, including serotyping and susceptibility testing, should be performed to establish the carrier state and the correct treatment. It is recommended that the prophylactic use of Mino Riemser be reserved for situations in which the risk of meningococcal meningitis is high.
Oral Mino Riemser is not indicated for the treatment of meningococcal infection.
Although no controlled clinical efficacy studies have been conducted, limited clinical data show that oral Mino Riemser hydrochloride has been used successfully in the treatment of infections caused by Mycobacterium marinum.
To reduce the development of drug-resistant bacteria and maintain the effectiveness of Mino Riemser® (Mino Riemser hydrochloride) Pellet-Filled Capsules and other antibacterial drugs, Mino Riemser® (Mino Riemser hydrochloride) Pellet-Filled Capsules should be used only to treat or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. When culture and susceptibility information are available, they should be considered in selecting or modifying antibacterial therapy. In the absence of such data, local epidemiology and susceptibility patterns may contribute to the empiric selection of therapy.
How should I use Mino Riemser?
Use Mino Riemser extended-release tablets as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- An extra patient leaflet is available with Mino Riemser extended-release tablets. Talk to your pharmacist if you have questions about this information.
- Take Mino Riemser extended-release tablets by mouth with or without food. Taking Mino Riemser extended-release tablets with food or milk may decrease stomach upset. However, certain foods and milk may bind with Mino Riemser extended-release tablets, preventing its full absorption. Discuss any questions or concerns with your doctor.
- Swallow Mino Riemser extended-release tablets whole. Do not break, crush, or chew before swallowing.
- Take Mino Riemser extended-release tablets with a full glass of water (8 oz [240 mL]). Do not lie down for 30 minutes after taking Mino Riemser extended-release tablets.
- Do not take medicines that have aluminum, magnesium, calcium, or zinc (eg, multivitamins, antacids, quinapril), bismuth, or urinary alkalinizers (eg, sodium bicarbonate) in them within 2 hours before or after you take Mino Riemser extended-release tablets.
- Do not take medicines that have iron (eg, ferrous sulfate) in them within 3 hours before or 2 hours after taking Mino Riemser extended-release tablets. If you need to take iron, check with your doctor about the right type of iron to take along with Mino Riemser extended-release tablets.
- Mino Riemser extended-release tablets works best if it is taken at the same time each day.
- Do not miss any doses. If you miss a dose of Mino Riemser extended-release tablets, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.

Off Label Uses
Cellulitis (purulent) due to community-acquired MRSA
Based on the Infectious Diseases Society of America (IDSA) Clinical Practice Guidelines for the Treatment of Methicillin-Resistant Staphylococcus Aureus Infections in Adults and Children, Mino Riemser is effective and recommended in the outpatient management of purulent cellulitis due to community-acquired methicillin-resistant S. aureus (MRSA).
Leprosy
Based on the National Hansen's Disease Program (NHDP) treatment recommendations, Mino Riemser, in combination with other agents, is an effective and recommended alternative agent for treatment of lepromatous (multibacillary) and tuberculoid (paucibacillary) leprosy.
Nocardiosis
Data from a limited number of patients studied suggest that Mino Riemser, with or without other concomitant antimicrobials, may be beneficial for the treatment of nocardiosis
Prosthetic Joint Infection
Based on the Infectious Diseases Society of America (IDSA) Diagnosis and Management of Prosthetic Joint Infection: Clinical Practice Guideline, Mino Riemser (based on in vitro susceptibility) given for indefinite chronic oral antimicrobial suppression of prosthetic joint infections is an effective and recommended treatment option. Mino Riemser is one of the preferred agents for Staphylococci (oxacillin-resistant) infections and is an alternative treatment for infections caused by Cutibacterium spp.
Rheumatoid arthritis
Data from older controlled studies and a meta-analysis have demonstrated a beneficial effect on laboratory parameters and modest clinical benefit in patients with rheumatoid arthritis (RA). Some data suggest that Mino Riemser may be effective in patients with recent-onset disease to reduce the total steroid dose needed.
The American College of Rheumatology (ACR) updated guideline for the treatment of RA did not include Mino Riemser due to its infrequent use in RA and lack of new data since prior publication.
Mino Riemser description
Mino Riemser hydrochloride is a yellow, crystalline powder; sparingly soluble or soluble in water, slightly soluble in alcohol, practically insoluble in chloroform and in ether, dissolves in solutions of alkali hydroxides and carbonates.
Mino Riemser hydrochloride is 4-7, bis (dimethylamino)-1, 4, 4a, 5, 5a, 6, 11, 12a-octahydro-3,10,12, 12a-tetrahydroxy- 1,11-dioxo-2-naphthacenecarboxamide monohydrochloride.
It has a molecular formula of C23H27N3O7 HCl. Molecular weight is 493.94.

The recommended dosage of Mino Riemser® is approximately 1 mg/kg daily for 12 weeks. Higher doses have not shown to be of additional benefit in the treatment of inflammatory lesions of acne, and may be associated with more acute vestibular side effects.

Mino Riemser® Tablets may be taken with or without food. Ingestion of food along with Mino Riemser® may help reduce the risk of esophageal irritation and ulceration.
In patients with renal impairment, the total dosage should be decreased by either reducing the recommended individual doses and/or by extending the time intervals between doses.

What other drugs will affect Mino Riemser?
Anticoagulants
Because tetracyclines have been shown to depress plasma prothrombin activity, patients who are on anticoagulant therapy may require downward adjustment of their anticoagulant dosage.
Penicillin
Since bacteriostatic drugs may interfere with the bactericidal action of penicillin, it is advisable to avoid giving tetracycline-class drugs in conjunction with penicillin.
Methoxyflurane
The concurrent use of tetracycline and methoxyflurane has been reported to result in fatal renal toxicity.
Antacids and Iron Preparations
Absorption of tetracyclines is impaired by antacids containing aluminum, calcium or magnesium and iron-containing preparations.
Low Dose
Oral Contraceptives
In a multi-center study to evaluate the effect of Mino Riemser on low dose oral contraceptives, hormone levels over one menstrual cycle with and without Mino Riemser 1 mg/kg once-daily were measured. Based on the results of this trial, Mino Riemser-related changes in estradiol, progestinic hormone, FSH and LH plasma levels, of breakthrough bleeding, or of contraceptive failure, cannot be ruled out. To avoid contraceptive failure, female patients are advised to use a second form of contraceptive during treatment with Mino Riemser.
Drug/Laboratory Test Interactions
False elevations of urinary catecholamine levels may occur due to interference with the fluorescence test.

- sores, ulcers, or white spots on the lips or in the mouth
- troubled breathing
- unusual bleeding or bruising

Some Mino Riemser side effects may not need any medical attention. As your body gets used to the medicine these side effects may disappear. Your health care professional may be able to help you prevent or reduce these side effects, but do check with them if any of the following side effects continue, or if you are concerned about them:

- difficulty with moving

What is the most important information I should know about Mino Riemser?
Do not use this medicine if you are pregnant. It could harm the unborn baby or cause permanent tooth discoloration later in life.
Mino Riemser can make birth control pills less effective. Ask your doctor about using a non hormone method of birth control (such as a condom, diaphragm, spermicide) to prevent pregnancy while using Mino Riemser.
Mino Riemser passes into breast milk and may affect bone and tooth development in a nursing baby. Do not take this medication without telling your doctor if you are breast-feeding a baby.
Children should not take Mino Riemser. Mino Riemser can cause permanent yellowing or graying of the teeth in children younger than 8 years old.
Avoid exposure to sunlight or tanning beds. Mino Riemser can make you sunburn more easily. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors.
Do not take iron supplements, multivitamins, calcium supplements, antacids, or laxatives within 2 hours before or after taking Mino Riemser. These products can make Mino Riemser less effective.
Take this medication for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antibiotics.
Throw away any unused medicine after the expiration date on the label has passed. Using expired Mino Riemser can cause damage to your kidneys.
