What is Powerflam PLUS?
Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID). It works by reducing hormones that cause inflammation and pain in the body.
Powerflam PLUS 1% gel (Voltaren
Topical
) is used to treat joint pain in the hands, wrists, elbows, knees, ankles, or feet caused by osteoarthritis. This medication may not be effective in treating arthritis pain elsewhere in the body.Powerflam PLUS 1.5% solution (Powerflam PLUS) is used to treat pain in the knees caused by osteoarthritis. This medication may not be effective in treating arthritis pain elsewhere in the body.
Powerflam PLUS 3% gel (Powerflam PLUS) is used to treat warty overgrowths of skin (actinic keratoses) on sun-exposed areas of the body.
Powerflam PLUS may also be used for purposes not listed in this medication guide.
Powerflam PLUS indications
Powerflam PLUS® (diclofenac sodium) Gel is indicated for the topical treatment of actinic keratoses (AK). Sun avoidance is indicated during therapy.
How should I use Powerflam PLUS?
Use Powerflam PLUS as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Powerflam PLUS comes with an extra patient information sheet called a Medication Guide. Read it carefully. Read it again each time you get Powerflam PLUS refilled.
- Do not apply Powerflam PLUS to infected skin; open wounds; or red, swollen, or peeling skin.
- Wash your hands immediately before and after using Powerflam PLUS.
- Wash the affected area and dry completely before using Powerflam PLUS.
- To apply Powerflam PLUS, dispense 10 drops at a time onto the knee or into your hand. Spread Powerflam PLUS evenly around the front, back, and sides of the knee. Repeat until you have used the whole dose.
- Be sure that you cover your entire knee with your dose of Powerflam PLUS. Do not use more than the recommended amount.
- Do not apply sunscreens, cosmetics, insect repellants, other topical medicines, or any other substance to the treated area until it is completely dry.
- Do not put on clothes over the treated area until it is completely dry.
- Do not wrap, bandage, or apply heat to the treated area.
- Let the treated skin dry before touching it or letting it touch anyone else's skin.
- Do not shower, bathe, or wash the treated area for at least 30 minutes after you use Powerflam PLUS.
- If you miss a dose of Powerflam PLUS, skip the missed dose and go back to your regular dosing schedule. Do not use 2 doses at once.

Uses of Powerflam PLUS in details
Powerflam PLUS is used locally in the treatment of painful conditions such as arthritis of joint (osteoarthritis), conditions of the joint arising from destruction of healthy body tissue by the body’s immune system (rheumatoid arthritis, ankylosing spondylitis), lower back pain, sprains or bone fractures, inflammation of joints (frozen shoulder, gout), pain and swelling over body.
Powerflam PLUS description
Powerflam PLUS tablet also contains the following excipients: Core: Magnesium stearate, povidone, anhydrous colloidal silica, sodium starch glycollate, maize starch, calcium phosphate.
Sugar Coating: Microcrystalline cellulose, polyethylene glycol 8000, red iron oxide (E172) and titanium dioxide (E171) (dispersed Anstead), povidone, talc, sucrose.
Polish: Polyethylene glycol 8000, sucrose.
Imprint: Brown printing ink for 25 mg and white for 50 mg.
Diclofenac potassium is potassium-[0-[(2,6-dichlorophenyl)-amino]-phenyl]-acetate.
In Powerflam PLUS, the sodium ion of diclofenac sodium (Voltaren) has been replaced by a potassium ion.

Powerflam PLUS® Gel is applied to lesion areas twice daily. It is to be smoothed onto the affected skin gently. The amount needed depends upon the size of the lesion site. Assure that enough Powerflam PLUS® Gel is applied to adequately cover each lesion. Normally 0.5 g of gel is used on each 5 cm x 5 cm lesion site. The recommended duration of therapy is from 60 days to 90 days. Complete healing of the lesion(s) or optimal therapeutic effect may not be evident for up to 30 days following cessation of therapy. Lesions that do not respond to therapy should be carefully re-evaluated and management reconsidered.

Intervention: | The concomitant use of diclofenac with other NSAIDs or salicylates is not recommended. Do not use combination therapy with Powerflam PLUS and an oral NSAID unless the benefit outweighs the risk and conduct periodic laboratory evaluations. |
Pemetrexed | |
Clinical Impact: | Concomitant use of Powerflam PLUS and pemetrexed may increase the risk of pemetrexed-associated myelosuppression, renal, and GI toxicity. |
Intervention: | During concomitant use of Powerflam PLUS and pemetrexed, in patients with renal impairment whose creatinine clearance ranges from 45 to 79 mL/min, monitor for myelosuppression, renal and GI toxicity. NSAIDs with short elimination half-lives (e.g., diclofenac, indomethacin) should be avoided for a period of two days before, the day of, and two days following administration of pemetrexed. In the absence of data regarding potential interaction between pemetrexed and NSAIDs with longer half-lives (e.g., meloxicam, nabumetone), patients taking these NSAIDs should interrupt dosing for at least five days before, the day of, and two days following pemetrexed administration. |

Clinical Trials Experience
Because clinical trials are conducted under widely varying conditions, adverse reaction ratesobserved in the clinical trials of a drug cannot be directly compared with rates in the clinical trials of another drug and may not reflect the rates observed in practice.
In controlled trials during the premarketing development of Powerflam PLUS, approximately 600 patients with minor sprains, strains, and contusions were treated with Powerflam PLUS for up to two weeks.
Adverse Events Leading To Discontinuation Of Treatment
In the controlled trials, 3% of patients in both the Powerflam PLUS and placebo patch groups discontinued treatment due to an adverse event. The most common adverse events leading to discontinuation were application site reactions, occurring in 2% of both the Powerflam PLUS and placebo patch groups. Application site reactions leading to dropout included pruritus, dermatitis, and burning.

Localized Reactions
Overall, the most common adverse events associated with Powerflam PLUS treatment were skin reactions at the site of treatment. Table 1 lists all adverse events, regardless of causality, occurring in ≥ 1% of patients in controlled trials of Powerflam PLUS. A majority of patients treated with Powerflam PLUS had adverse events with a maximum intensity of “mild” or “moderate.”

Foreign labeling describes that dermal allergic reactions may occur with Powerflam PLUS treatment. Additionally, the treated area may become irritated or develop itching, erythema, edema, vesicles, or abnormal sensation.

What is the most important information I should know about Powerflam PLUS?
Do not use this medication if you have ever had asthma or a severe allergic reaction caused by aspirin, diclofenac (Powerflam PLUS, Voltaren), or another non-steroidal anti-inflammatory drug (NSAID). Do not use diclofenac just before or after having heart bypass surgery (also called coronary artery bypass graft, or CABG).
Before using this medication, tell your doctor if you have heart disease, congestive heart failure, high blood pressure, a history of heart attack or stroke, a history of stomach ulcer or bleeding, liver or kidney disease, a blood clotting disorder, asthma, or nasal polyps.
While the risk of absorbing Powerflam PLUS into your bloodstream is low, an NSAID may cause life-threatening heart or circulation problems such as heart attack or stroke, especially if you use it long term.
Get emergency medical help if you have chest pain, weakness, shortness of breath, slurred speech, or problems with vision or balance.
This medicine may also cause serious effects on the stomach or intestines, including bleeding or perforation (forming of a hole). These conditions can be fatal and can occur without warning while you are using Powerflam PLUS, especially in older adults.
Call your doctor at once if you have symptoms of stomach bleeding such as black, bloody, or tarry stools, or coughing up blood or vomit that looks like coffee grounds.
