Pregnancy of Rythma in details

sponsored

Rythma and the active metabolite, N-desethylamiodarone, cross the placenta (Plomp 1992).

In utero exposure may cause fetal harm. Reported risks include neonatal bradycardia, QT prolongation, and periodic ventricular extrasystoles; neonatal hypothyroidism (with or without goiter); neonatal hyperthyroxinemia; neurodevelopmental abnormalities independent of thyroid function; jerk nystagmus with synchronous head titubation; fetal growth retardation; and/or premature birth.

Oral or IV Rythma should be used in pregnancy only to treat arrhythmias refractory to other treatments or when other treatments are contraindicated (ACC/AHA/HRS [Page 2015]).

Rythma (administered either maternally or directly to the fetus) may be considered for the in utero management of fetal atrial flutter and in life-threatening cases of sustained fetal supraventricular tachycardia refractory to first and second line agents, but because of potential toxicity, risks and benefits should be assessed (AHA [Donofrio 2014]; Kang 2015).

If in utero exposure occurs, newborns should be monitored for thyroid disorders and cardiac arrhythmias.
