Pharmacology: Cipra Pro is a selective serotonin (5HT) re-uptake inhibitor used for the treatment of depression and panic disorder.
Mechanism of Action: The pharmacological and clinical effects of Cipra Pro are attributed to 5HT reuptake inhibition. It has low affinity for a number of receptors including 5HT1A, 5HT2, D1 and D2 receptors, α1, α2, β-adrenoreceptors, histamine, muscarinic, benzodiazepine and opioid receptors.
Pharmacokinetics: The absorption of Cipra Pro is almost completely independent of food intake. As with racemic citalopram, the absolute bioavailability of Cipra Pro is expected to be 80%. Following a single dose of Cipra Pro 20 mg, the mean Tmax was reported to be 5±1.5 hrs. Steady-state plasma levels are reached in about 1 week.
Cipra Pro is metabolised in the liver to the demethylated and didemethylated metabolites, both of these are pharmacologically active. Biotransformation of Cipra Pro to the demethylated metabolite is mediated primarily by CYP2C19 with some possible contribution by the enzymes CYP3A4 and CYP2D6. The plasma protein-binding is <80% for Cipra Pro and its main metabolites.
The elimination t½ after multiple dosing is about 30 hrs and the oral plasma clearance is about 0.6 L/min. Elimination of Cipra Pro and the major metabolites are assumed to occur both by the hepatic and the renal routes, with the major part of the dose excreted as metabolites in the urine.
The elimination of Cipra Pro is more slow in elderly patients as compared to younger patients. Systemic exposure (AUC) is about 50% higher in elderly compared to young healthy volunteers.
How should I take Cipra Pro?
Take Cipra Pro only as directed by your doctor to benefit your condition as much as possible. Do not take more of it, do not take it more often, and do not take it for a longer time than your doctor ordered.
Cipra Pro should come with a Medication Guide. Read and follow these instructions carefully. Ask your doctor or pharmacist if you have any questions.
Cipra Pro may be taken with or without food. If your doctor tells you to take it at a specific time, follow your doctor's instructions.
If you are using the oral liquid, shake the bottle well before measuring each dose. Use a marked measuring spoon, oral syringe, or medicine cup to measure each dose. The average household teaspoon may not hold the right amount of liquid.
You may have to take Cipra Pro for a month or longer before you begin to feel better.
Dosing
The dose of Cipra Pro will be different for different patients. Follow your doctor's orders or the directions on the label. The following information includes only the average doses of Cipra Pro. If your dose is different, do not change it unless your doctor tells you to do so.
The amount of medicine that you take depends on the strength of the medicine. Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine.
- For oral dosage forms (solution or tablets):
- For depression:
- Adults and children 12 years of age and older—10 milligrams (mg) once a day, taken either in the morning or evening. Your doctor may adjust your dose as needed. However, the dose is usually not more than 20 mg per day.
- Older adults—10 mg once a day, taken either in the morning or evening.
- Children younger than 12 years of age—Use and dose must be determined by your doctor.
- For generalized anxiety disorder:
- Adults—At first, 10 milligrams (mg) once a day, taken either in the morning or evening. Your doctor may adjust your dose as needed. However, the dose is usually not more than 20 mg per day.
- Older adults—10 mg once a day, taken either in the morning or evening.
- Children—Use and dose must be determined by your doctor.
- For depression:
Missed Dose
If you miss a dose of Cipra Pro, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.
Storage
Store the medicine in a closed container at room temperature, away from heat, moisture, and direct light. Keep from freezing.
Keep out of the reach of children.
Do not keep outdated medicine or medicine no longer needed.
Ask your healthcare professional how you should dispose of any medicine you do not use.

Cipra Pro pharmacology
Cipra Pro is one of a class of antidepressants known as selective serotonin reuptake inhibitors (SSRIs). It is used to treat the depression associated with mood disorders. It is also used on occassion in the treatment of body dysmorphic disorder and anxiety. The antidepressant, antiobsessive-compulsive, and antibulimic actions of Cipra Pro are presumed to be linked to its inhibition of CNS neuronal uptake of serotonin. In vitro studies show that Cipra Pro is a potent and selective inhibitor of neuronal serotonin reuptake and has only very weak effects on norepinephrine and dopamine neuronal reuptake. Cipra Pro has no significant affinity for adrenergic (alpha1, alpha2, beta), cholinergic, GABA, dopaminergic, histaminergic, serotonergic (5HT1A, 5HT1B, 5HT2), or benzodiazepine receptors; antagonism of such receptors has been hypothesized to be associated with various anticholinergic, sedative, and cardiovascular effects for other psychotropic drugs. The chronic administration of Cipra Pro was found to downregulate brain norepinephrine receptors, as has been observed with other drugs effective in the treatment of major depressive disorder. Cipra Pro does not inhibit monoamine oxidase.
