Diclamin indications
For the treatment of functional disturbances of gastrointestinal motility eg, irritable bowel syndrome and renal and biliary colic. As with other antispasmodics, Diclamin has limited efficacy in the treatment of these disorders and should be used only if other measures (eg, diet) have been of little or no benefit.
Diclamin has been used alone or in combination with phenobarbital in the treatment of infant colic and acute enterocolitis but lacks substantial evidence of efficacy in these conditions. Infant colic is considered a benign and self-limiting condition.
Diclamin description
A muscarinic antagonist used as an antispasmodic and in urinary incontinence. It has little effect on glandular secretion or the cardiovascular system. It does have some local anesthetic properties and is used in gastrointestinal, biliary, and urinary tract spasms.
Diclamin dosage
Oral
Gastrointestinal tract spasm
Adult: 10-20 mg tid.
Child: 2-12 yr: 10 mg tid; 6 mth-2 yr: 5-10 mg 3-4 times daily.
Elderly:
Intramuscular
Gastrointestinal tract spasm
Adult: 20 mg 4 times a day given only for 1-2 days. Replace with oral therapy as soon as possible.
Diclamin interactions
The following agents may increase certain actions or side effects of anticholinergic drugs. amantadine antiarrhythmic agents of class (e.g. quinidine), antihistamines antipsychotic agents (e.g. phenothiazines), benzodiazepines. MAO inhibitors, narcotic analgesics (e.g., meperidine), nitrates and nitrites, sympathomimetic agents, tricyclic antidepressants, and other drugs having anticholinergic activity.
Anticholinergics antagonize the effects of antiglaucoma agents. Anticholinergic drugs in the presence of increased intraocular pressure may be hazardous when taken concurrently with agents such as corti costeroids..
Anticholinergic agents may affect gastrointestinal absorption of various drugs, such as slowly dissolving dosage forms of digoxin; increased serum digoxin concentrations may result.
Anticholinergic drugs may antagonize the effects of the drugs that alter gastrointestinal motility, such as metoclopramide. Because antacids may interfere with the absorption of anticholinergic agents, simultaneous use of these drugs should be avoided.
The inhibiting effects of anticholinergic drugs on gastric hydrochloric acid secretion are antagonized by agents used to treat achlorhydria and those used to test gastric secretion.
Diclamin side effects
Dryness of mouth w/ difficulty in swallowing & talking, thirst, reduced bronchial secretions, mydriasis w/ cycloplegia & photophobia, flushing & dryness of the skin, transient bradycardia w/ palpitations & arrhythmias, difficulty in micturition, constipation.
Diclamin contraindications
Diclamin should not be given to patients with obstructive uropathy, obstructive disease of the GIT, ulcerative colitis, reflux esophagitis, hypersensitivity to Diclamin HCl and nursing mothers.
